OMS Employment Application

W4 Form (2011)

Why become a part of OMS?
OMS is dedicated to providing our patients with sound judgment, compassionate care, and achieving an excellence in our education. Our clients choose our services because of our professionalism, our ability to adapt to their needs, and our high standard of medical care. There are many reasons for our clients to request our presence; some benefit in terms of insurance premiums, some are required to because of local laws, and others are simply concerned with the health of their patrons or employees. Our services consist of stand-by medical care at events, which include but are not limited to: Night Clubs, Private Parties, Corporate Events, Concerts, and medical care at industrial job sites. At each event, employees will be provided with everything they need to administer Basic Life Support as described under the pretenses of being a certified EMT.

Our Medical Staff have access to one of our Medical Board physicians at times. We require that our employees maintain professionalism at all times. Employees are required to arrive on time to the event they are scheduled. They must have a personal appearance that keeps with the OMS Employee Handbook and EMT-B Guidelines. Employees must have clean/ polished steel-toed boots, navy blue class A EMT pants with reflectors, and a navy blue OMS Uniform Shirt (provided by OMS). Jackets, pants, boots and other gear can be purchased through the company at a discounted price, but will not be provided. Employees must maintain a positive attitude and demeanor when dealing with clients or patients, as they are not only representing OMS but they will be representing our clients as well.

OMS Employees will benefit from constant instruction, monthly training sessions at our facility, a family atmosphere, knowledgeable and supportive management, and access to a wide range of EMS experience. In addition, OMS Medical Staff receive 10% off their AT&T service plans!
